Birth Registration

ALL birth registrations for births which occurred after March 30th, 2009 at 12:00 Midnight, will be processed by the Office of the Registrar General.  Documentation will be provided to the parent(s) of the child at the time of the birth by the Royal Victoria Hospital or appropriate Health Professional.

Parents may submit their registration through the following methods:

  • Electronically:   www.ServiceOntario.ca/newborn
  • Mail-In:  Submit the completed form to The Office of the Registrar General, Box 4600, 189 Red River Road, Thunder Bay, ON  P7B 6L8

Registering your child’s birth is the first step to giving your baby a legal identity.  Once your baby’s birth is registered with the Province of Ontario, you will be able to apply for a birth certificate. The birth certificate is issued by the Province of Ontario and must be applied for separately.

Birth Certificate Applications 

The Birth Certificate application form is available online at www.cbs.gov.on.ca or www.serviceontario.ca or at any Land Registry Office.  The Land Registry office in Barrie is located at 114 Worsley Street.  Applications are also available for pick up at the City of Barrie, Clerk’s Office at 70 Collier Street. 

The City of Barrie does not accept or process birth certificate applications. The application form must be submitted to the Provincial Government Office of the Registrar General, Ministry of Consumer and Business Services, P.O. Box 4600, 3rd Floor, 189 Red River Road, Thunder Bay, Ontario P7B 6L8. Options for submitting a completed application are indicated on the application form. 

The Provincial Office of the Registrar General offers the following service options for processing applications:

  • Online applications – Clients are now required to provide a guarantor reference, rather than a guarantor signature. Payment for online applications must be made by credit card. When completed, the form is sent electronically to the ORG. Birth certificate applications submitted online will be processed and delivered in 15 business days. The Government Information Centre located in 34 Simcoe Street has a computer for the public use.
  • Regular service requires that the application and appropriate fee be mailed to Office of the Registrar General, Ministry of Consumer and Business Services, P.O. Box 4600, 3rd Floor, 189 Red River Road, Thunder Bay, Ontario P7B 6L8 or faxed to 1-807-343-7459.
  • Expedited service is available with proper proof of urgency and requires 10 days for processing. The application must be faxed to 1-807-343-7411.
  • Emergency service is available only at the Office of the Registrar General in Toronto at 900 Bay Street, 2nd Floor, Room M2-49.  This service requires 2 full business days with proof of urgency, excluding delivery.  Hours of operation are 8:30 a.m. to 5:00 p.m.
